What you need to know
- If you have submitted an ESRA form already through the DOMNI portal, don’t worry, your information has been transferred to the new system.
- The move to DUO means fewer systems to navigate and a more seamless user journey.
- It is only the Leading Investigator on-site (experiment responsible), that needs to submit the ESRA form.

Will this affect my upcoming experiment?
No. Existing submitted ESRAs have been transferred, and new ESRA form submissions can be completed directly in DUO.

I submitted a risk assessment in DOMNI already. Do I need to submit it again?
No. Existing ESRA forms has been transferred to DUO, and no further action is required.
Will my ESRA forms from previously performed experiments still be accessible?
Yes. Information from previously submitted ESRA forms has been migrated to DUO.
How do I access ESRA in DUO?
Log in to DUO and navigate to the risk assessment section. Detailed instructions are available in the user guide.
Do I need a new account to access the ESRA in DUO?
No. You should continue using your existing DUO credentials.
What if I started filling out an ESRA in DOMNI but did not submit it before 5 October?
In this case you need to complete a new one in DUO.
Will the risk assessment process itself change?
All communication will now mainly be through DUO and/or User Office, not via emails from the Experimental Safety Team. However, the information required in the ESRA form remains largely the same.
I would like to add samples to my experiment. Previously I had to add them in DUO and the ESRA form, how should I do now?
When you initiate a new ESRA form for your experimental session, the ESRA will be prefilled with all data from DUO. If you would like to add new samples to your experiment you can do that in the ESRA form. The ESRA form will automatically update your DUO sample information.
What if I submitted the ESRA form and would like to make changes?
You can request to make changes in the risk assessment section in DUO directly.

Contact the User Office or the support contact listed in DUO.
I am part of an experiment but not the main responsible for the experiment. Do I have to submit an ESRA form?
No, only the Leading Investigator on-site (experiment responsible) needs to submit the ESRA form.
